In order to apply for a Florida Medical Marijuana Identification Card, you must first obtain a signed Physician Certification Form from a certified physician. The purpose of evaluation is to determine whether you can move forward in the process.

While that answer varies by state and is ever-changing, Amendment 2 for the Use of Marijuana for Debilitating Medical Conditions outlines that the following conditions/diseases may be treated with the use of medicinal cannabis:

- Muscular Dystrophy
- Parkinson’s Disease
- Sickle Cell Anemia
